APA and MLA format help.?

K, I have a huge project due very soon. I asked my mom for help and she helped me out, I have to write the source, how would I do it in MLA or APA format, what would I put. Would it be her or would it be her teacher? My teacher told me that it would fall under interview but what information do I put? Please help, Thanks.
